WORKSHOP REGISTRATION INFO - READ THOROUGHLY
The following pages are the requirements and registration form for the upcoming Biomedx Biotorium workshop.
DO NOT book your flight until you are confirmed as being in the class. We will send you an email to confirm your seat.
If you get the confirmation email, the program is happening unless stated otherwise. Confirmation of your seat will generally be sent out within a day or two but at the beginning of registration for a class it could be 7 to 14 days while we gauge registrations coming in. Sometimes classes start slow, sometimes they fill very fast. Seats fill as registration forms are received, our seating is limited.
When we receive your faxed-in form we expect that if you are paying by check you will send the hard copy with payment promptly. If funds are not received in the following week you risk being displaced in the program.
(Sending the payment after the form is faxed saves us from having to chase it down. Sometimes people say they are coming and they fax in the form and then they don’t send payment, they change their plans, and they fail to tell us. That’s not good. If paying by cash or coin [gold/silver], call the office to make those arrangements .)
Once confirmed, the email confirming your spot will go out with a link to a document with the details of the workshop times, addresses and hotel info along with pre-workshop study information and how to access it. Lunch is also provided for each full program day.
Important: You will need to call the hotel directly and book your room to get our rates with the covered ground transportation. Our rate varies depending on days & season, avg is $90-140 per night.
Briefly: Programs are in Chicago/Des Plaines, 2 miles due north of Chicago O’Hare International Airport, airport code ORD. (Midway airport is on the south side of Chicago, 45 minutes away on a good day. You will need a car if flying there.) Our guest hotel is the Best Western Des Plaines Inn.
The Biotorium is 5 blocks from the hotel. Ground transportation to and from O’Hare airport and the workshop location is provided by the hotel itself. All details will be in the follow-up email when your registration is confirmed.

Hotel pick up each day is 20 minutes prior to class start time. Shuttle return time to the hotel is 10 minutes after class end time. To review the class agenda for your program, go to http://biomedx.com/training.
Workshops are private events, not open to the public.
We have put pre-class study material for you online. You will get information on this and your access password shortly after registration. The amount of material there is extensive.
Consider it mandatory to review before attending the live class. People generally need 2-4 full days for this review.
There are video sessions from prior programs you need to watch.
There is workbook material you need to read, and study.
If you don’t get into this material and study it, you will be at great disadvantage attending the workshop.
EXTENDED STAY OPTION
Following the 4 day workshop that ends Sunday, you may be able to do continuing study with our resident unregistered nurse at her location in Naperville, Illinois, on Monday morning. This is flex time to do your own personal blood audit in depth, go through more of the scope work and dietary/nutrition guidelines with clients, intern for a day or two, etc. $500 per day. This will follow the formal class and can be however many days you want. A change of hotel Sunday to Naperville is warranted for this. This practicum is space limited to the first few students who might want that.
If you want to pursue this option, write EXTENDED STAY on top of your registration form and you will be contacted prior to the workshop to make those arrangements.
